Billing Information
An initial Biopsychosocial is scheduled for (1) 1/2 hours for adults; as well as, for children with their parents. Clients of Minor age (state law dependent) will require at least one parent present during the Initial Biopsychosocial Assessment, and proper consents for treatment, from either parent, must be signed. We meet with both parents and child together at first, then the child alone, and will see the parents and child altogether at the end of the appointment to discuss how to go forward with treatment. The frequency of follow-up appointments will depend on the progress of each client and individual progress on their specific treatment plan.
South Florida Health Coalition apologizes for the inconvenience; however, our office does not accept insurance at this time, which means we are an out of network provider. All Clients are required to pay in full for their appointments at the time of each visit. After each follow-up the Client will be provided with an insurance claim form for the purpose of submitting them, on their own, to their insurance companies for potential reimbursement, should they decide to claim the office visits on individual insurance.
